An inquest has been opened into the death of Captain Philip Gilbert Muldowney, aged 25, who tragically lost his life on January 25th. The incident occurred during a live firing training exercise at the Otterburn Training Area, located in Northumberland.
Senior coroner Andrew Hetherington detailed that Captain Muldowney sustained a gunshot wound during the exercise at Otterburn Camp. CPR was administered by military medics, and ambulance services attended, but he was pronounced deceased at the scene.
Investigations into the circumstances surrounding Captain Muldowney's death are being carried out by Northumbria Police, the Health and Safety Executive, and the Defence Safety Authority, which is conducting a service inquiry.
Captain Muldowney, who grew up in County Roscommon, Ireland, was a fire support commander. The Ministry of Defence remembered him as an "immensely enthusiastic officer" who inspired those around him. He joined the Army in January 2020 and commissioned into the Royal Artillery in December 2020.
