CharmHealth and PointClickCare have launched a new partnership designed to facilitate real-time, bidirectional data exchange between their respective healthcare platforms. This integration specifically targets long-term post-acute care (LTPAC) facilities, addressing long-standing interoperability challenges that often create detrimental gaps in patient health information.

The collaboration ensures that patient data, including diagnoses, demographics, medications, and insurance details, are automatically synchronized from PointClickCare into CharmHealth, maintaining consistency across systems. Furthermore, chart notes finalized in CharmHealth are instantly sent back to PointClickCare, ensuring all care teams remain updated on patient status.

This advancement is set to streamline workflows by minimizing administrative burdens and potential errors, which historically have led to rework and billing denials. By enhancing transparency and coordination, the partnership aims to significantly improve care transitions and patient safety within post-acute settings.
