Staff Bullying Alleged at Unit Where Teen Died
14 Jul
Summary
- A teenager died at a mental health unit in 2024.
- An internal report detailed staff allegations of bullying and unprofessional conduct.
- The unit's failures contributed to the teenager's death, an inquest found.

The mother of a teenager who died at a mental health unit in 2024 has questioned whether her daughter would still be alive had concerns about staff bullying been addressed sooner. Lucy, aged 17, passed away on January 1, 2024, after being found unresponsive at the Riverside Adolescent Unit in Bristol on December 27, 2023. An inquest found that the unit's failures, including missed patient checks and delayed medical aid, "probably contributed" to her death.
An internal review, not presented to the inquest jury, has since highlighted allegations of bullying and "unprofessional communication" among staff in the months surrounding Lucy's death. The report indicated that staff described a lack of empathy from senior leaders and judgmental comments about young patients. This has deeply upset Lucy's family, who question if improvements in staff conditions could have prevented the tragic outcome.
The Avon and Wiltshire Mental Health Partnership NHS Trust (AWP), which operated the unit, has apologized for instances where conduct fell below expected standards. They stated that measures to address these cultural problems have been identified and will be integrated into a planned replacement specialist centre. The trust emphasized that such unacceptable behavior does not reflect the values of the majority of their staff.
