This program provides crucial support, including visits from nurses, physical therapists, and social workers, tailored to individual needs. Hutchison, who experienced significant weakness after her hospital stay, found the flexible support essential, stating that the program helped her avoid returning to the hospital and regain independence.
The 'LHSC to Home' program, funded by the province and delivered by SE Health, has enrolled over 500 patients. Managers hail it as a 'game-changer,' noting its success in improving patient recovery and alleviating pressure on hospital resources by freeing up beds for those who require inpatient care.
