The global call for 'breastfeeding is best' overlooks critical implementation challenges, with Pakistan facing significant funding gaps. Evidence shows skilled counseling can raise exclusive breastfeeding rates by 58 percent, a service ideally delivered by the nation's Lady Health Workers (LHWs). However, this vital network is severely understaffed and underfunded, hindering effective nationwide reach and monitoring.

Furthermore, the Baby Friendly Hospital Initiative (BFHI), proven to increase breastfeeding rates, has seen its certifications lapse over time. Formalizing lactation support within disaster response protocols is also urgent. Breastfeeding offers resilient support during climate-induced disruptions, unlike formula feeding, which requires scarce resources like clean water.

Integrating trained counselors and safe spaces for mothers into disaster management is essential. Building breastfeeding capacity among vulnerable populations before crises strike is more effective than attempting to establish it during emergencies. Strengthening these backend systems of funding, monitoring, and implementation is paramount to truly support a sustainable start in life.
