University Hospitals of Morecambe Bay NHS Foundation Trust has announced a substantial reduction in hospital readmissions following the implementation of the Integrated Wellness Model of Care. This initiative, launched in 2025, focuses on identifying and supporting patients with chronic conditions such as heart failure and diabetes who are at a higher risk of returning to hospital shortly after discharge.
The model fosters close collaboration between hospital services, social care, and community health teams. By working together, these services can proactively implement measures to prevent unnecessary readmissions. This integrated approach has proven highly effective in managing patient recovery outside the hospital setting.
Data from the trust reveals a dramatic success: out of 151 patients flagged as high-risk since the program's inception, only 11% were readmitted within eight weeks. This marks a significant improvement from the previous rate of 75% recorded in 2023, demonstrating the initiative's positive impact on patient outcomes and healthcare efficiency.
