A 27-year-old woman died by misadventure in August 2025, shortly after a distressing phone call with an NHS 111 service worker. She detailed her plan to the handler, who assured her that help was being arranged and that the property's door was open.
Despite the explicit suicidal intent expressed by Ms. Loraine, the call handler permitted her to end the call, stating they could disconnect once help was in place. The handler advised her to call back if her condition worsened. Tragically, Ms. Loraine died minutes later, before assistance could arrive. The call concluded at 5:31 AM, and attempts to call her back by a clinician between 5:40 AM and 5:45 AM were unsuccessful.
The Senior Coroner for Sunderland has issued a Prevention of Future Deaths Report, highlighting a critical gap in national NHS guidelines. The coroner noted that the telephone triage system provides no specific guidance on maintaining contact with patients expressing suicidal intent. This report has been sent to NHS England, requiring a response by May 27.
