The initial recommendation for immediate jeopardy came from the North Carolina Department of Health and Human Services, citing unsafe patient environments.
The North Carolina Department of Health and Human Services identified critical failures, including inadequate systems for patient misidentification, a lack of adherence to telemetry escalation pathways, and insufficient monitoring during patient transport. These issues placed patient health and safety at risk. The hospital staff had also faced public attention, with nurses organizing a rally in September to highlight ongoing staffing and safety challenges.
Following a CMS survey on November 7, the immediate jeopardy designation was lifted. Mission Health President Greg Lowe expressed appreciation to CMS and hospital staff for their diligence, emphasizing a commitment to providing high-quality care. The health system plans to submit an updated plan of correction by January 15, 2026, and undergo a re-survey, while maintaining existing action plans for identified issues.
