- Primary care spending target set at 15% by 2036.
- AI use in utilization review faces new oversight.
- Rebates must pass to patients, lowering drug costs.

House Democrats took a significant step forward by advancing a comprehensive primary care reform package on Wednesday. The proposed legislation includes new regulations for artificial intelligence in healthcare, strengthens oversight of pharmacy benefit managers, and aims to improve access to public health services.
A key feature of the bill is the establishment of primary care spending targets, with the goal of reaching 15% of total healthcare expenditures by 2036. This initiative seeks to bolster reimbursement rates, expand the primary care workforce, and reduce administrative burdens on providers.
The bill also introduces important consumer protections, notably regulating the use of AI in utilization reviews. Automated tools cannot be the sole basis for adverse determinations, requiring a licensed physician's review. Furthermore, it mandates that at least 80% of prescription drug rebates be passed on to patients at the pharmacy counter, lowering out-of-pocket costs.
Additional measures include a $25 million investment in workforce development, insurance parity for community health centers, and an expedited review process for certain expansions of health services in underserved communities.
