Families affected by severe harm or death during maternity and neonatal care have welcomed the publication of the terms of reference for a new inquiry as a significant milestone. This comprehensive review, officially launched by the Department of Health and Social Care (DHSC), will meticulously examine individual cases.
The inquiry is set to investigate the delivery of maternity and neonatal care from 1 January 2011 until 31 March 2028, focusing on instances where mothers and babies suffered severe harm or death while under the care of LTHT. Clinical case reviews are slated to commence in November, with the overall review expected to conclude by 2029.
Senior midwife Donna Ockenden, who previously led a similar inquiry in Nottingham, will head this crucial review. The DHSC specified that eight categories of severe harm or death will be considered. The review aims to identify existing concerns and formulate recommendations to enhance safety, quality, and equity in maternity and neonatal services.
