India: Hip Replacements Face New Complications
2 Jun
Summary
- Infections and fractures are rising causes for revision hip surgery.
- Young patients with demanding lifestyles are experiencing complications.
- Aseptic loosening remains common but its contribution is declining.

A comprehensive 15-year study conducted at two major Indian hospitals reveals a notable shift in the reasons for revision hip replacement surgeries. Infections and fracture-related complications are increasingly cited as primary drivers for these complex procedures, moving beyond aseptic loosening. This trend is particularly observed in younger patients with active lifestyles.
The research, analyzed 252 revision total hip arthroplasties between 2011 and 2025. While aseptic loosening, where implants detach without infection, continues to be the most frequent cause, its relative proportion has diminished. Simultaneously, infections, instability, and fractures around the implant are becoming more prominent.
Modern hip replacement surgery continues to offer excellent outcomes for most patients. However, the increasing number of both young and elderly individuals undergoing joint replacement, coupled with longer lifespans, naturally leads to a rise in the overall volume and complexity of revision surgeries over time.
Medically vulnerable patients, especially those with multiple uncontrolled comorbidities, face a higher risk of complications, including post-operative infections. Careful pre-operative medical optimization, addressing conditions like diabetes and cardiovascular disease, is crucial for improving patient outcomes and mitigating infection risks.
Advancements in reconstruction options, such as India-manufactured 3D-printed implants and customized revision systems, are enhancing the management of severe bone loss and complex revision hip surgeries. These technologies improve surgical effectiveness, accessibility, and affordability for patients in India.
Long-term follow-up after initial hip replacement is vital for early detection of potential issues like infection or loosening, which can prevent complications. Ongoing improvements in implant design, surgical techniques, infection control, and perioperative care continue to enhance the durability and success of hip replacements.
