Daughter's Final Wish: Hospital Failings Exposed
25 Jun
Summary
- Hospital observation checks were reduced despite self-harm disclosures.
- An inquest found missed opportunities significantly contributed to her death.
- The daughter's final wish was to expose failings in her care.

A recent inquest into the death of Rianna Poiana-Lazarec revealed significant failings at Sherwood Oaks mental health hospital. Rianna, who had a history of self-harm and diagnoses of EUPD and ADHD, was admitted in November 2024. Her mother, Andreea Lazarec, recounted how Rianna's wish to attend Cambridge was impacted by her mental health struggles. Initially placed on high observation, Rianna's checks were later reduced to hourly. Despite disclosing suicidal intentions and self-harming 23 times in 23 days, access to a self-harming item was not restricted.
The jury concluded that Rianna did not intend to end her life and that the reduced observations and inadequate control of a household item significantly contributed to her death. They found multiple opportunities were missed to prevent her passing. A spokesperson for Nottinghamshire Healthcare expressed deep sorrow and stated changes have been implemented since Rianna's death. Rianna's mother shared that her daughter's final wish was to highlight these care deficiencies to help others.
