A case report published in August 2026 in the International Journal of Research in Medical Sciences detailed a 57-year-old woman with type 2 diabetes who developed persistent gastrointestinal issues while on semaglutide. After four months of epigastric pain, loss of appetite, and weight loss, she was diagnosed with eosinophilic gastroenteritis. Symptoms and blood eosinophil counts normalized after the drug was discontinued.
This report is supported by a nationwide Danish registry analysis. Researchers found that hypereosinophilia was approximately three times more likely after starting a GLP-1 drug compared to before. This signal was specific to GLP-1s, with comparator diabetes drugs showing no similar association.
While GLP-1 drugs are known for common side effects like nausea, this case and others suggest a rare hypersensitivity reaction. Reports have emerged globally linking GLP-1 use to eosinophilic conditions in the esophagus, duodenum, stomach, and colon. These findings emphasize the importance of a higher index of suspicion for atypical gastrointestinal symptoms in patients using these medications.
Doctors are advised to consider drug-induced eosinophilic disorders in patients on GLP-1s who present with persistent or unusual gastrointestinal complaints. Early detection and drug withdrawal can lead to complete recovery. Patients experiencing prolonged pain, significant appetite loss, or unexplained weight loss should consult their healthcare provider, as these symptoms differ from typical drug side effects.
