Despite Lisa's persistent concerns about Ophelia's cough over a three-month period, medical professionals repeatedly assured her that her daughter's chest sounded fine. The toddler tragically died on February 22, 2025, with a post-mortem later revealing pneumonia as the cause of death.

Ophelia had been experiencing a cough that came and went, and after one incident where she coughed up blood, she was taken to A&E. While deemed playful and fine, no further tests like a chest X-ray were conducted.
