- Roller coaster safety bar reportedly required multiple attempts to secure.
- Deceased had pre-existing spinal injuries and used a wheelchair.

A fatal roller coaster incident at Orlando's Epic Universe theme park on September 17 has concluded with investigators ruling the death of Kevin Rodriguez Zavala as accidental. His girlfriend, Javiliz Cruz-Robles, reported that she screamed for help as the tragedy unfolded during the Stardust Racers ride, but her cries went unheard until the cart approached the station.
Investigations revealed that the safety bar for the ride reportedly required multiple attempts to secure properly. During the ride, Kevin allegedly flew forward, striking his head multiple times. Cruz-Robles stated she was unable to hold him back and her pleas for assistance were initially unheeded.
Further details emerged indicating Kevin had a pre-existing spinal cord injury and a history of hip and femur issues, which his parents confirmed. Despite these factors, investigators determined theme park employees followed standard operating procedures, leading to the case being closed as an accidental death.
