Assisted Living Facility Shut Down Over Patient Death Allegations

An assisted living facility in Hialeah, Florida, has been issued an Emergency Suspension Order, mandating its closure by 5 p.m. on Friday, July 31, 2026. The facility, Our Dream ALF on Palm Avenue, must relocate its 26 residents. The Florida Agency for Healthcare Administration (AHCA) has accused staff of severe neglect.
Allegations detailed in the order include the failure to administer necessary medications to a resident who was later found unresponsive on July 26. According to AHCA, staff also failed to perform CPR appropriately, did not use an automated external defibrillator, and delayed calling 911 for the resident who passed away.
Further investigations revealed other instances of neglect. In April, a resident was discovered on the floor with a significant amount of blood. The AHCA also found four months' worth of unused psychotropic medications belonging to a resident stored in a shower.
