An 89-year-old resident, Marion Villavecchia, died on May 1, 2026, after an alleged assault by a fellow resident at Extendicare Heritage Lodge. She was found on April 18, 2026, with a bleeding nose and head injuries, reporting the man had punched her. Medical reports revealed she suffered a broken nose, facial swelling, and lost teeth in the days following the incident. Concerns were raised by her friend, Ivan Master, about whether staff acted quickly enough, as Villavecchia's condition worsened significantly over five days before she was examined by a doctor.
The Protection for Persons in Care Office concluded its investigation late last month, determining it was "more likely than not" that Villavecchia experienced abuse and neglect. The facility received 10 directives aimed at improving documentation, staff education, and oversight of resident behaviors. Extendicare acknowledged gaps in managing safety incidents and is working to fulfill the directives, including enhanced training and monitoring. The Winnipeg Regional Health Authority has also launched a critical incident review, and the facility remains under licensing restrictions preventing new admissions without approval.
