In a bittersweet turn of events, Lisa Vanderpump's 28-year-old nephew Sam Vanderpump has announced that he and his fiancée, Alice Yaxley, are expecting their first child. The couple shared the pregnancy news on September 16, 2025, just weeks before Sam revealed his diagnosis of end-stage liver disease.
"Life is full of surprises, but this is the best one yet," Sam captioned the announcement, which included a photo of the ultrasound scan and the couple's hands. Sam and Alice, 24, told The Daily Mail that the pregnancy was a "big shock" but that they were excited to welcome their baby boy, whom they plan to name Marmaduke, after Sam's late father.
However, the couple's joy has been tempered by Sam's health struggles. On October 27, 2025, Sam shared on the UK reality show Made in Chelsea that he has been diagnosed with end-stage liver disease and is awaiting a transplant assessment. "There is no hope of my liver getting better," he said, adding that doctors have told him he may not have more than four or five years to live without a transplant.
Despite the difficult news, Sam remains hopeful, saying, "The way I look at it, I'm healthy now. Hopefully I remain healthy all the way up until one day [when] they'll give me a call, and I'll go in for a surgery. And then I wake up and I'll be healthier."
