Summary
- A 69-year-old with kidney issues underwent complex angioplasty.
- Advanced rotablation technology successfully cleared a severe blockage.
- The procedure minimized contrast use to protect kidney function.
A complex angioplasty procedure was successfully performed on a 69-year-old patient experiencing acute coronary syndrome and concurrent kidney dysfunction. The patient was admitted to the ICU due to severe chest pain. Coronary angiography revealed a significant, calcified blockage in the left anterior descending artery.
Given the patient's pre-existing renal issues, traditional bypass surgery presented increased risks. Consequently, doctors opted for a rotablation-assisted percutaneous coronary intervention (PCI). This advanced technique utilizes a high-speed rotating burr to modify hardened calcium deposits within the artery, enabling effective balloon angioplasty and stent placement.
During the procedure, approximately 16 rotablation runs were completed without complications. A subsequent cutting balloon treatment and the placement of a drug-eluting stent successfully restored blood flow. Crucially, efforts were made to limit contrast dye usage, employing a low volume of visipaque to safeguard kidney function. The entire intervention was performed under local anesthesia, minimizing patient invasiveness.
