This lack of psychological safety is a key finding, as it can hinder the identification and resolution of risks.
The CQC also noted "limited assurance of learning from governance processes to mitigate risk." While nursing staff provided positive feedback on direct management, the overarching leadership team was advised to lead by example more effectively. The trust must now focus on embedding a robust culture of learning and improvement throughout its operations.
