Tammy Mackenzie, aged 41, passed away at Heartlands Hospital in August 2023, just days after her daughter, Khloe, was stillborn. An inquest into Tammy's death determined that critical delays in diagnosis likely led to the tragic outcome for both mother and child.
Mackenzie had been admitted to the hospital multiple times since May 2023, experiencing abdominal pain and vomiting. A recommendation for an MRI scan was made on July 26, but it was not carried out until July 30. This scan revealed a severe small bowel obstruction.
Despite treatment, Tammy's condition deteriorated, leading to emergency abdominal surgery at 28 weeks pregnant. Following the surgery and an emergency C-section for her daughter, Tammy's health continued to decline. She passed away on August 6, with her cause of death attributed to multiple organ failure and a strangulated small intestine.
