These citations stem from an incident where a resident suffered a broken hip, a torn wrist ligament, and went into shock after a nursing assistant improperly attempted to lift them using a mechanical device. State health codes mandate two assistants for such transfers when a resident cannot assist themselves, a rule allegedly violated in this case.
This event, which occurred on September 29, 2025, resulted in the resident's hospitalization for ten days. The facility was cited for failing to keep the patient free from abuse, neglect, and accident hazards. This latest citation is part of a recurring pattern, with the facility having received numerous citations in recent months, including 19 in January and additional ones in November and December of the preceding year. Officials have not commented on the citations, but inspection reports suggest staffing shortages may be a contributing factor to these ongoing safety issues. Facilities across Pennsylvania are currently required to meet new minimum staffing levels for direct resident care.
