Previously, under the Transplantation of Human Organs and Tissues Act (THOTA) 1994, kidney swap donations were restricted to simple two-way exchanges between two incompatible donor-recipient pairs. This limitation often prevented successful transplants due to issues like ABO blood group mismatches.
The new Karnataka Multi-Pair Kidney Exchange Transplantation Guidelines 2026 allow for the linking of three or more donor-recipient pairs. This innovative approach enables donors incompatible with their own relatives to donate a kidney to another patient within the group, while their relative receives a compatible kidney from elsewhere in the group.
The guidelines detail the necessary approval mechanisms, mandatory documentation, and ethical safeguards to ensure the safe and effective implementation of these multi-pair kidney swaps. This initiative is expected to significantly increase transplant opportunities for eligible patients in Karnataka.
