Bihar's TB Drive: Proactive Screening Saves Lives

India is undergoing a significant public health transformation with its TB Mukt Bharat Abhiyaan, actively seeking out individuals for TB screening rather than waiting for them to present symptoms. Bihar has embraced this mandate with exceptional success, screening almost twice its target of 10 million people by August 14th.

This intensified screening is crucial as nearly half of TB cases are asymptomatic, contributing to silent transmission. Bihar's incidence remains above the national average, making expanded diagnostic tools vital for early detection. This effort builds on Patna's previous success as a pilot city for engaging private healthcare providers in quality TB care, a model now scaled nationally.

Prime Minister Narendra Modi's personal commitment to TB elimination since 2018 has galvanized national efforts. The private sector is now a key partner, with mandatory reporting of TB cases leading to over 35% of notifications coming from private practitioners. Direct cash transfers for nutritional support have also been mandated for TB patients since 2018.

These combined strategies have yielded impressive results, with India recording a 21% reduction in TB incidence and a 25% fall in TB mortality since 2015, significantly outpacing global declines. The spirit of Jan Bhagidari, or public participation, is evident with over 700,000 Ni-kshay Mitras offering support. Eliminating TB requires a comprehensive social response, fostering empathy and reducing discrimination, with all ministries contributing to address vulnerability factors.

Science is also advancing, with new portable and non-invasive diagnostic tools on the horizon. However, the ultimate success in closing the distance to elimination hinges on national resolve, a quality clearly demonstrated across Bihar and the country.
