India's journey in liver transplantation, beginning with a successful procedure in 1998, has seen exponential growth. From a few hundred transplants annually in the mid-2000s, the country now conducts approximately 4,500 such surgeries each year, positioning itself among Asia's leaders. This transformation is fueled by medical breakthroughs, including minimally invasive techniques and expanded transplant types, alongside policy support and the development of skilled medical professionals.

The advancement in liver transplantation is underpinned by significant innovation. Surgical techniques have evolved, and essential immunosuppressant drugs are now produced domestically, drastically reducing long-term treatment costs. The growing expertise has enabled a wider range of complex procedures, such as ABO incompatible and multiorgan transplants. Furthermore, training programs have cultivated a new generation of specialists, extending transplant services beyond major cities.

Despite these achievements, critical challenges remain. India's organ donation rate is still low compared to global averages, necessitating a shift towards deceased donor transplants. Enhancing public awareness and streamlining donor registration are crucial steps. Expanding health insurance coverage and improving early patient referral systems are also vital to ensure greater financial inclusivity and better outcomes for those needing liver transplants.
