India's Diabetes Model: A Global Health Blueprint

India has developed a robust, low-cost, and tech-enabled framework for diabetes management over four decades, positioning itself to guide other low and middle-income nations. The strategy prioritizes indigenous innovations over imported solutions, advocating for integrated care and acknowledging the unique characteristics of diabetes in South Asia. Diabetes in this region manifests differently, often appearing at lower BMIs with increased visceral fat and insulin resistance, necessitating tailored screening and treatment approaches that consider lifestyle and environmental factors like diet and physical activity.

The "total diabetes care" model, pioneered at Chennai's Dr. Mohan's Diabetes Specialities Centre, redefines care by addressing the comprehensive impact of diabetes and associated metabolic disorders. This integrated approach streamlines services such as screening, treatment, education, lab work, and specialist consultations for eyes, kidneys, feet, and nutrition. Such integration enhances care delivery efficiency, minimizes treatment fragmentation, and reduces patient delays.

To combat specialist shortages, the model champions "task shifting," training physicians, diabetes educators, and technicians to extend care beyond tertiary and secondary settings. Technology plays a pivotal role in ensuring continuity of care, with recommendations including electronic medical records, AI chatbots, patient apps, telemedicine for services like ophthalmology, and decision-support tools for classifying diabetes subtypes. This shift aligns with precision medicine's personalized approach, moving away from one-size-fits-all treatments.
