These systemic issues contributed to residents not being kept safe and experiencing undignified treatment, including significant delays in receiving assistance for basic needs like using the toilet.
Inspectors also noted a decline in personal hygiene among residents, with some family members observing their relatives in soiled clothing. Critical errors were found in care planning, posing direct risks. For example, one diabetic resident did not consistently receive food after insulin, contrary to their care plan. Another resident with swallowing difficulties was inappropriately given sandwiches, increasing their choking risk.
