The Windsor Regional Hospital (WRH) has broadened its Hospital to Home initiative, a program designed to prevent readmissions and emergency room visits for recently discharged patients. This significant expansion, which took effect on April 27, 2026, now welcomes patients from Erie Shores HealthCare and Hôtel-Dieu Grace Healthcare into the program.
Launched initially in October 2025 with $2 million in provincial funding, Hospital to Home offers comprehensive support for eight to 16 weeks. Services such as nursing, personal support, dietitian, and physiotherapy are managed by SE Health, a not-for-profit social enterprise, ensuring a streamlined and personalized approach compared to standard provincial care.
SE Health providers collaborate closely with WRH, conducting follow-up calls and adjusting care plans as needed within the community. The program's objective is to onboard 15-20 patients daily, with a new enrollment target of 399 patients by the end of March 2027. This initiative is crucial in reducing the number of alternative level of care (ALC) patients awaiting discharge.
WRH CEO Kristin Kennedy highlighted that partnering with Erie Shores and Hôtel-Dieu enhances the program's broader impact by reducing hospital admissions and ER visits. The hospital continues to collaborate with its partners and Ontario Health at Home to ensure smooth patient transitions and effective rehabilitation at home.
