District health authorities in Mohali are set to launch a one-year pilot project for home-based palliative care. This initiative aims to enhance community healthcare by providing compassionate, doorstep services to patients with chronic or terminal illnesses who are home-bound.
The project focuses on improving patients' quality of life through coordinated, multidisciplinary care. Each patient will receive visits from a dedicated palliative care team, including medical interns, community health officers, dentists, physiotherapists, counselors, and lab technicians, ensuring comprehensive support.
This program intends to bridge gaps in end-of-life and chronic care, reduce unnecessary hospital visits, and offer dignity and comfort to patients at home. If this model proves successful, it may be expanded for replication across Punjab.
