Summary
- A 78-year-old received a complex TAVR-in-TAVR-in-SAVR procedure.
- Doctors implanted a new valve inside two previous failing valves.
- The procedure utilized bioprosthetic valve fracture and cerebral protection.

In a remarkable feat of medical innovation, doctors at Kauvery Hospital, Alwarpet, Chennai, successfully completed a TAVR-in-TAVR-in-SAVR procedure on January 22, 2026. The patient, a 78-year-old resident, had a history of aortic valve disease and multiple prior interventions.
The procedure was intricate due to a bicuspid aortic valve and an enlarged main heart artery. Complicating matters further were the failures of a previously implanted transcatheter valve and a surgical valve. Open-heart surgery was deemed too high-risk for the patient.
A multidisciplinary team utilized advanced imaging to plan and execute the transcatheter solution. A new valve was implanted within the existing structures. Bioprosthetic valve fracture was employed to enhance valve expansion and blood flow, alongside dual cerebral protection devices to mitigate stroke risk.
This successful local management of a complex case highlights advancements in cardiac care and reduces the need for patients to seek treatment abroad.
