A new provincial initiative, CKHA @ Home, has been launched in Chatham-Kent, backed by an annual investment of $1.3 million. This program is designed to streamline the process for patients transitioning from hospital stays back to their homes. It also seeks to alleviate pressure on in-patient hospital beds, which are currently experiencing significant demand.
Hospital officials noted a substantial increase in patients requiring beds, with the facility operating approximately 22 beds over capacity. The CKHA @ Home program will assign a dedicated patient navigator to assist patients and their families in developing individualized discharge care plans. These plans may incorporate services from personal support workers, occupational therapists, nurses, or social workers, facilitating a smoother return home and promoting quicker recovery.
This annual funding is projected to benefit around 160 patients each year, offering services for eight to sixteen weeks depending on individual needs. The initiative is expected to improve patient outcomes by enabling them to heal in a home environment. Beyond the initial support period, the program will also help connect patients with ongoing community services if required, ensuring continued care and support.
