Antibiotic Powder Combo Fails Bone Surgery Test
20 Apr
Summary
- Extra antibiotic powder offers no additional infection protection.
- Infection rates were similar with one or two antibiotic powders.
- Vancomycin alone remains a solid option for specific infections.
A clinical trial involving over 1,500 patients across 39 U.S. trauma centers has found that using a combination of antibiotic powders does not reduce post-surgery infection rates for complex bone fractures. The study, published on April 15, 2026, randomly assigned patients to receive either vancomycin powder alone or a combination of vancomycin and tobramycin powders. Results indicated no significant difference in infection rates between the two groups, with 7.4% of those receiving the combination and 6.6% of those receiving vancomycin alone experiencing infections.
Researchers noted that vancomycin targets gram-positive bacteria, while tobramycin targets gram-negative bacteria. Despite the theoretical advantage of broader coverage, the addition of tobramycin did not lead to a decrease in infections. Principal investigator Dr. Robert O'Toole stated that the belief that adding an aminoglycoside like tobramycin would reduce gram-negative infections did not hold true. This suggests that vancomycin alone remains an effective strategy for preventing gram-positive infections, and the routine addition of tobramycin powder is not supported by current data.
