In August of last year, 29-year-old Sam Dudley lost his life at the Hoggs Hill level crossing in Formby. While running and wearing headphones, Dudley entered the crossing as warning lights flashed and a klaxon sounded.
CCTV footage revealed Dudley appearing shocked as a train approached, attempting to jump back moments before impact. An investigation confirmed he entered the track while signals indicated danger. His phone was found displaying a warning about loud music volume.
The senior coroner concluded that Dudley likely did not hear the klaxon due to listening to loud music, leading to distraction and inattentiveness. The crossing itself was found to be safe and operating as designed, with no fault in its inspection or maintenance.
However, the coroner identified concerns regarding warning signage at the gate, suggesting clearer visual warnings like crossed-out earphones could improve pedestrian awareness. A combination of visual and auditory cues was recommended to enhance safety communication at such crossings.
