A Dodge Caravan, operated by Nancy Heldman of Findlay, was traveling westbound. According to reports, a Buick Ranier, driven by Kenneth Alan Griggs, 18, of Lima, exited a gas station and entered the eastbound lane. Griggs then crossed into the westbound lane, resulting in a head-on collision with Heldman's vehicle.

Heldman and a passenger, Stacy Trimble, 50, also from Findlay, were transported to Memorial Health System. Griggs suffered minor injuries and was cited for being left of center and driving with a temporary permit. He also could not provide proof of insurance at the scene.
