A devastating workplace accident claimed the life of a 55-year-old employee at Specialty Rolled Metals LLC in Carol Stream on Tuesday. The incident unfolded inside the S&S International warehouse as the worker assisted a truck driver in loading a large metal spool, estimated to weigh around 48,000 pounds.
Authorities reported that a component of the crane system, possibly a strap, failed. This failure caused the heavy spool to fall directly onto the employee, resulting in immediate fatality. The Carol Stream Fire District responded to the scene just after noon.
Both the Carol Stream Police Department and the Occupational Safety and Health Administration (OSHA) have launched investigations into the tragic event. OSHA arrived on scene before 4 p.m. to commence their inquiry into the workplace fatality.
