A coroner has determined that the death of Father Paddy O'Kane in March 2022 was preventable. Father O'Kane, 73, was being treated for depression at Gransha Park hospital in Londonderry when he died. While the coroner found his diagnosis and general observation levels to be appropriate, she noted missed opportunities in his care. The lack of a dedicated risk assessment for the garden area allowed for the presence of ligature materials, ultimately contributing to his death.
The Western Health Trust has accepted the coroner's findings, apologizing for its failings and stating that remedial actions based on Serious Adverse Incident recommendations are in place. The trust has assured that the hospital garden is now included in all safety assessments to prevent a recurrence.
