Manitoba's chief medical examiner has ordered inquests into three separate in-custody deaths occurring between July 2022 and March 2026. These investigations will scrutinize the circumstances surrounding each fatality.
One inquest will focus on Jeremy Peters, 42, who died on July 9, 2022. Peters was taken into custody under the Intoxicated Persons Detention Act and was later found unresponsive in his cell. An autopsy determined his death was accidental, caused by a combination of cocaine, methamphetamine, and ethanol.
Further inquests have been called for Craig Flett, 32, and Sebastian Atkinson, 25. Flett died by suicide on February 10, 2026, at Stony Mountain Institution. Atkinson also died by suicide on March 25, 2026, at the Winnipeg Remand Centre. Both were found hanging in their cells.
These inquests are mandated under The Fatality Inquiries Act and will aim to recommend measures to prevent similar deaths in the future. Dates for these proceedings have not yet been set.
