Bihar's Prohibition Paradox: Cough Syrup Fuels New Addiction
30 May
Summary
- Codeine cough syrups are a cheap substitute for alcohol in Bihar.
- Addiction cases are rising among youngsters in rural areas.
- Seizures of codeine syrup bottles have significantly increased since 2016.

Bihar's strict alcohol prohibition, in place for nearly a decade, has led to an alarming rise in the misuse of codeine-based cough syrups, with youngsters and rural populations disproportionately affected. This substance has become a cheap and accessible substitute for alcohol, leading to increased seizures and addiction cases across the state. The Directorate of Revenue Intelligence recently intercepted 7,500 bottles of such syrup in Patna.
Codeine, an opioid derived from morphine, is primarily used as a cough suppressant. However, its misuse can lead to serious side effects, including drowsiness, respiratory depression, addiction, and long-term damage to organs and mental health. The intoxicating effects, especially when combined with other depressants, make it a dangerous alternative to alcohol.
Since the prohibition was enforced in 2016, the illicit market for codeine syrups has thrived, with bottles often available for INR 100-200. Seizures have surged dramatically, from under 30,000 bottles in 2016 to over 1.17 lakh bottles by 2024. Authorities are grappling with this, as police resources are strained by both the ongoing illegal liquor trade and the trafficking of narcotics like codeine syrups.
Despite being classified as 'intoxicants' under Bihar's Prohibition and Excise Act, enforcement remains challenging due to weak pharmacy oversight and the ease of black market distribution. While stricter measures are being considered, experts suggest addressing underlying issues like poverty and unemployment is crucial for long-term solutions. The situation presents a complex dilemma between public health concerns and access for legitimate medical needs.
