The child, Sarthak Yadav, was admitted for leukemia treatment and tragically passed away on December 17, 2025. An internal inquiry revealed gross negligence in handling a hazardous chemical, formalin.
During a scheduled procedure on December 16, 2025, a syringe containing formalin was allegedly left accessible after the procedure was deferred. The following morning, a nursing officer reportedly used this syringe to flush the child's IV line without confirming its contents, ignoring the father's warnings.
The child immediately lost consciousness and was moved to the PICU, where resuscitation efforts failed. He was declared dead later that morning. The AIIMS investigation committee reportedly linked the death directly to the formalin administration. Police have registered cases for causing death by negligence and negligent handling of poisonous substances.
