US Healthcare Spending Hits Historic High, Surpasses All Categories
27 Feb
Summary
- Federal healthcare spending is now the largest category of US expenditures.
- Medicare costs are projected to double by 2036, straining budgets.
- Medicare's Hospital Insurance Trust Fund faces exhaustion by 2040.

Federal healthcare spending has become the largest category of US expenditures, surpassing Social Security, national defense, and interest payments. Current trajectories suggest health programs will consume an enormous share of the national budget for years to come. Over $26 trillion is projected for major health programs through 2036.
Medicare costs are expected to double from $988 billion in 2025 to nearly $2 trillion by 2036. Medicaid and the Children's Health Insurance Program spending will rise by 36%, and Affordable Care Act subsidies by a third. This growth critically impacts the Medicare Hospital Insurance Trust Fund, now predicted to be exhausted by 2040, 12 years sooner than previously estimated.
This fiscal crisis is attributed to higher medical costs and reduced revenue from tax cuts impacting Social Security benefits. Despite these projections, President Trump has pledged to protect Medicare and Social Security. However, the Social Security trust fund could be depleted as early as 2031.
Healthcare spending accounts for 30% of all projected spending growth through 2036. Policy recommendations include standardizing service payment rates and cracking down on Medicare Advantage overpayments. Without intervention, the government faces difficult choices between increased debt or severe cuts to essential benefits.
