The initiative, which Schwarzenegger founded in 2017 as part of the Austrian World Summit, focuses on critical climate issues such as pollution and sustainable food practices. Schwarzenegger expressed deep admiration for Milligan, a physical therapist. He shared that their relationship began after he was referred to her for shoulder surgery rehabilitation in 2012. Following his recovery and completion of the film "Escape Plan," Schwarzenegger reached out to Milligan to express his gratitude, leading to a lunch date that marked the start of their relationship.

While Milligan often maintains a lower public profile compared to Schwarzenegger's industry appearances, she has been seen with him at significant events. These include a press conference for Climate Justice in Vatican City last September and the "FUBAR" season 2 premiere in Los Angeles in 2023. The couple also attended Oktoberfest in Munich in September 2022, underscoring their shared experiences and Schwarzenegger's appreciation for her independent and driven spirit.
